As the Antibirth devs have said, it used a lot of C++ workarounds that wouldn't work with AB+, meaning a lot of the code is being remade to be compatible. They slowly walk towards Isaac, occasionally firing a tear at Isaac when damaged. The Binding of Isaac: Repentance is a randomly generated action/RPG/shooter with heavy roguelike elements, resulting in many thousands of hours of gameplay. Just because Antibirth didn't require AB+ doesn't mean that Repentance won't.
